Staff Forms & Links

How to use the forms on this page:

1. Find the link to the form/area you would like to access and click on the link. If the link takes you to another page, click on the form you want to access.
2. Click on the document you need in the folder and it will pop up. If it is a paperless form*, you can complete the form entirely online, if it is a fillable** (non-paperless form) see step 3, if it is not a fillable or paperless form, Print.
3. If it is a fillable form, download the document. (In Chrome, the arrow that points down with the line under it just above the document)*

*Paperless forms will open and be submitted in Informed K12, the district's electronic form platform

**All fillable forms must be opened in Adobe Acrobat to work correctly.  If the form is not opening in Adobe but is opening in the web browser, right click on the form in your downloads folder and select "Open With" then choose "Adobe Reader".
