Many of the forms for Benefits are now available ONLINE through Informed K-12. You may choose either to download the pdf and fill out the form to return to Benefits, OR fill out the form online through the link to Informed K-12.  

Health Insurance Waiver 2026

This form is for those who are entitled to the medical insurance benefit for the 2025-26 benefit year, but choose not to elect due to existing coverage. They will receive a one-time in-lieu payment, dispersed in December of 2026.
